Our professional staff is here to take your referral 24 hours a day, seven days a week. All you have to do is make the call.
Following a brief telephone interview, staff will assist you in scheduling an in-person assessment or make other recommendations to ensure your needs are met in the most appropriate and timely manner.
When calling to make a referral, you will be asked for the following information.
- Your name, phone number and relationship to referred individual.
- Referred individual's name, date of birth, social security number, address and phone number.
- A description of the referred individual's current problems.
- A date when the referred individual may be available for assessment.
Following a phone interview, clinical data will be discussed with a psychiatrist who determines the most appropriate disposition to meet the needs of the individual.
