To register for an inpatient or outpatient procedure, including
pre-admissions for surgery, visit the Floyd registration desk located
to the right directly inside the Main Entrance on West Sixth Street.

Upon arrival for admission, you will be asked to complete a Admissions Form. You may print the Admission Form if you would like to complete it before your admission date.
What to Bring for Admission
- Physician's order for procedure
- Proof of insurance
- Photo identification
- Medical history, including family medical history
- Co-payment, if applicable
- Admission Form
- List of prescription and non-prescription medications you are
currently taking
What to Expect
A member of the Floyd family will greet you at the registration
desk. To begin your admission, your information and paper work will
be collected. This will include questions about you and your family's
medical history, reason for admission, method of payment, insurance
coverage and doctor's information. After registration, you will
be escorted to the proper location for your procedure.
Financial Arrangements
Your hospital bill will include hospital charges only.
Other professional services preformed by health care providers — including
physicians, hospitalist, anesthesiologist and radiologist — will be
billed separately.
Charges not paid by your insurance coverage will be your responsibility. Payment methods include cash, personal check and the following credit cards: Discover, VISA and Mastercard.
